Course Description

The shoulder is designed for a great degree of motion in all planes. This mobility comes at the cost of stability as the shoulder is the joint at greatest risk for instability due to athletically-induced trauma. Shoulder injuries can be devastating in athletes, particularly in those whose sport requires strong overhead motions, and account for about 6% of all injuries sustained in an active population. Athletes also have a higher risk for recurrence, making proper diagnosis, treatment and rehabilitation crucial.

Series Outline:

Shoulder Instability

This section of the Shoulder Series examines the surgical anatomy of shoulder instability and reviews diagnosis, examination and imaging. Treatment options and rehabilitation recommendations are made for common injuries.

Total runtime: 1 hour

Acromioclavicular Injuries

This section of the series focuses on surgical and non-surgical options for acute and chronic injury to the AC joint.

Total runtime: 35 minutes

Rehabilitation of Selected Shoulder Conditions

This section focuses on clinical implications of evaluative aspects and procedures for the shoulder complex and examines treatment techniques and exercises. Also discussed are current concepts in strengthening of the shoulder complex musculature.

Total runtime: 40 minutes

Evaluation and Treatment of Scapular Dyskinesia

This session begins with a basic anatomical review of the shoulder-scapula complex before delving into functional anatomy, arthrokinematics, and surgery used to address multi-directional instability.

MRI of the Shoulder

This section of the Shoulder Series guides the sports medicine professional in an exploration of the anatomy of the upper shoulder and the normal imaging appearance. The professional will learn to identify major muscle groups of the upper extremity, modalities used in imaging and the connection between the anatomy and common injuries.

Upper Extremity Biomechanics & Rehab Principles

This session guides the sports medicine professional on identifying biomechanical risk factors for upper extremity injuries in youth athletes & aging populations as well as prevention and rehabilitation strategies.

Neurovasuclar Problems of the Throwing Shoulder

This concluding section focuses on the anatomy of the suprascapular nerve and possible compression sites and offers up clinical manifestations and surgical treatment of suprascapular nerve compression, quadralateral space syndrome, axilliary artery thrombosis, and thorasic outlet syndrome.

This course is intended to guide the professional through a study of the shoulder.

Professional Objectives:

At the conclusion of this course, participants will be able to

  1. Discuss treatment options for acute and chronic injury to the AC Joint.
  2. Recognize the anatomy of the shoulder and the normal imaging appearance.
  3. Identify the bones of the upper extremity.
  4. Recognize the major muscles and muscle groups of the upper extremity.
  5. Identify the modalities used in imaging the upper extremity.
  6. Recognize the anatomy as it pertains to some of the more common injuries to the upper extremity.
  7. Recognize the anatomy of the suprascapular nerve and possible compression sites.
  8. Cite the clinical manifestations of suprascapular nerve compression, quadralateral space syndrome, axilliary artery thrombosis, and thorasic outlet syndrome.
  9. Recognize the surgical treatment of suprascapular nerve compression, quadralateral space syndrome, axilliary artery thrombosis, and thorasic outlet syndrome".
  10. Identify biomechanical risk factors for upper extremity injuries in  Youth athletes & aging populations.
  11. Cite prevention strategies for various upper extremity injuries in youth athletes aging populations.
  12. Cite rehabilitation strategies for various upper extremity injuries in youth athletes & aging populations.
  13. Recognize the clinical implications of multiple evaluative aspects and procedures for the shoulder complex.
  14. Cite reliable treatment techniques in treatment of the stiff shoulder.
  15. Recognize effective exercise and strengthening progression rationale.
  16. Discuss current concepts in strengthening of shoulder complex musculature.
  17. Recognize basic anatomical/ biomechanical of the shoulder-scapula complex.
  18. Discuss functional anatomy and arthrokinematics of the clavicle, glenohumeral (GH) joint and scapula.
  19. Identify surgery to address multi- directional instability.

Criteria for Completion:

A score of 70% or more is considered passing.  Scores of less than 70% indicate a failure to understand the material and the test will need to be taken again until a passing score has been achieved.

Authors:

​Charles A. Roth, M.D.

Charles A. Roth MD, has over 25 years of experience as an orthopaedic surgeon. He is active in clinical research at the Andrews Institute, currently involved in the research of Cartilage Auto Graft Implantation (CAIS) for knee cartilage defects. In addition, Dr. Roth is the Principal Investigator in high molecular weight Hyaluronic Acid Injections for arthritis patients. He has participated as the Principal Investigator for a study concerning anti-coagulation after total knee replacement with Factor X inhibitors to prevent blood clots.

Dr. Charles A. Roth is a Clinical Assistant Professor of Orthopaedic Surgery at the University of South Alabama. He is currently a Clinical Assistant Professor for Florida State University and Clinical Assistant Professor for The University of South Alabama Physician Assistant Program.

Charles Leddon, ATC, PhD

Charles Leddon has been the Sports Medicine Research Associate at the Andrews Research and Education Foundation for two and a half years, where he oversees the biomechanics lab, fellow’s research projects, and manages the research initiatives for the physicians of the Andrews Institute. He did his undergraduate educational work at Pepperdine University in sports medicine, where he also traveled with the national championship baseball team as an athletic trainer (1992), and did his master’s level work in biomechanics as well as his doctoral work in sports medicine Oregon State University.

Charles has been a certified Athletic Trainer for 23 years, including 18 years in professional baseball and currently serves as the AA athletic trainer for the Cincinnati Reds in conjunction with his position at AREF.  His clinical experiences have included work in international sports with the US luge team and Australian national baseball team, the collegiate level with Pepperdine, Oregon State, and St. Mary’s College of Maryland, as well as work at the high school level and in multiple PT clinical settings during his career.

Chris Dorvault, MD

Dr. Dorvault earned his medical degree from Tulane University in New Orleans, Louisiana.  He completed his residency at the University of Pittsburgh and a Fellowship in Muscular Skeletal from Washington University in St. Louis, Missouri.  He has also served for four years in the U.S. Air Force, stationed at Maxwell AFB in Montgomery, Alabama. He is board certified in Diagnostic Radiology.

He is a member of several local and national organizations, including the Radiology Society of North America, the American College of Radiology, the Society of Skeletal Radiology, Escambia County Medical Society, the Florida Medical Society, the American Medical Society and the Florida Radiology Society.

Christopher O'Grady, MD

Christopher O'Grady, MD, is a board certified orthopaedic surgeon with a Certificate of Added Qualifications in Sports Medicine. He has a special interest in shoulder surgery and has served as an instructor in arthroscopic surgery at multiple national orthopaedic conferences.

After residency, the United States Navy assigned Christopher to the Pensacola Naval Hospital where he proudly served as an active duty orthopaedic surgeon for four years. While on active duty, he served as the orthopaedic consultant for the Blue Angels.

Dr. Christopher O'Grady is a consultant to NASA for the prevention of shoulder injuries in astronauts. He also serves as an associate master -instructor at the Orthopaedic Learning Center in Chicago for courses on shoulder instability and on preservation of cartilage in the knee.

Hunter Stark, DPT

Hunter Stark, DPT, is a physical therapist at the Andrews Institute’s rehabilitation center. He is an active speaker at the Andrew Institute’s annual Injuries in Football Conference as well as Sports Medicine Fellowship conference where he reinforces the importance of rehabilitation of shoulder injuries. 

Prior to joining the team at the Andrews Institute, Stark was a staff physical therapist at Mississippi Sports Medicine and Orthopaedic Center, Jackson, MS, and Baptist Health Systems, Jackson, MS. 

He graduated from Mississippi State University where he earned his Bachelors in Biological Sciences, completed his masters and Doctoral programs in Physical Therapy at the University of MS Medical Center.  He is a licensed physical therapist in the state of Florida.

James Hughey, DO

Dr. Hughey is an orthopaedic surgeon and a member of The Orthopaedic Center Sports Medicine Team. Before joining TOC, Dr. Hughey completed a fellowship in Sports Medicine at Andrews Research and Orthopedic Institute in Gulf Breeze, FL where he served as team physician for area high schools and served as assistant team physician for Pensacola's Cincinnati Reds AA baseball team. Dr. Hughey graduated Magna Cum Laude from Mississippi State University in Biochemistry. After graduation, he served as a research technician in the Department of Cell Biology and Neuroscience before moving to the Department of Orthopaedic Surgery at the University of South Alabama. Dr. Hughey completed his medical degree and orthopaedic residency at Kansas City University of Medicine and Biosciences.

Dr. Hughey has always been an active member of the community whether on the field as a medical volunteer, performing physicals for school children, providing medical care for the homeless or serving on medical education committees. He is certified in Advanced Cardiac Life Support, Advanced Trauma Support and Basic Cardiac Life Support.

Robert McCabe, MS, DPT, OCS

Robert McCabe is an APTA certified orthopedic specialist with fifteen years of clinical experience. He has extensive experience in the clinical evaluation of and development of treatment modalities for orthopedic and sports injuries. Bob holds an MS in Physical Therapy and an MS in Biomechanics and Ergonomics. He is currently completing his Ph.D. in Biomechanics and Ergonomics at New York University. Bob earned his DPT from Arcadia University.

Bob has coordinated pre-season strength and flexibility testing as well as physical screenings for world-class athletes. He has worked for both the Andrews Institute for Orthopedics and Sports Medicine and the Nicholas Institute for Sports Medicine as a Senior Physical Therapist.

Bob has served as a clinical educator for students at New York and Long Island Universities. He is published in the Journal of Orthopedic and Sports Physical Therapy as well as the North American Journal of Sports Physical Therapy. Bob McCabe has presented sports medicine seminars and research studies at national conferences including the APTA annual Combined Sections meeting and state association meetings.
